The state has 53 counties but only 12 licensed psychiatrists outside the Fargo and Bismarck metro areas. Mobile clinical teams must cover an average 68-mile radius between service points, with winter road closures extending response times beyond 90 minutes in 14 western counties.
Workforce data indicate 1.8 licensed clinical social workers per 10,000 residents in rural service areas versus 7.4 in urban counties. Existing telehealth infrastructure reaches only 38 percent of the most remote census tracts due to terrain and tower placement. These constraints limit consistent follow-up for patients requiring medication management or crisis intervention.
The funding requires applicants to maintain two-way radio or satellite communication for teams operating beyond cellular coverage. Capital supports vehicle outfitting and portable diagnostic equipment; operations funding covers 30 months of staffing with a required 1:3 match from county or tribal health budgets. Quarterly utilization and outcome reports are mandatory.
Infrastructure Requirements for North Dakota
Proposals must include signed agreements with at least two critical-access hospitals for referral pathways and real-time bed availability data. Equipment must operate in temperatures down to -30°F.
